-Not otherwise going for thrombectomy

-Within 24 hours of last known normal

-NIHSS 4 or more with disabling deficits

-CTP required, core <20 cc

-Mismatch volume of at least 10 cc

-HCT without established hypodensity that is consistent with recent infarct
